How to Check Your Insurance Coverage

We are in network with Blue Cross Blue Shield (Blue Choice Preferred, Blue Choice Options, Blue Choice PPO, and PPO plans) and United Healthcare (PPO plans). In general, most of these plans offer preventive diet counseling with $0 out of pocket cost and no deductible or diagnosis requirement. 

If insurance coverage is a concern, we recommend that you call your insurance company to ask about your coverage.

What is my responsibility (deductible, copay, coinsurance)?

Do I have any nutritional counseling benefits covered under the preventative care portion of my plan? 

 Is there a visit limit?

Is telehealth covered?
